Liability Waiver & Release of Liability *
I understand that participation in the internship program involves activities at various employer worksites. I voluntarily agree to participate and acknowledge that participating organizations, their staff, partner agencies, and host employers are not liable for any injury, loss, or damage that may occur during the course of the internship, except in cases of gross negligence or willful misconduct. I agree to follow all workplace rules, safety guidelines, and conduct expectations set by my host employer and the program. I understand that failure to do so may result in removal from the program. I acknowledge that program staff and host employers may need to share information about my participation for program coordination, reporting, and compliance purposes, including documentation required for workforce funding (such as WIOA).

Attendance & Punctuality — I will report to my internship site on time and as scheduled, and notify my supervisor and program coordinator as soon as possible if I cannot attend. Professional Conduct — I will treat my host employer, coworkers, customers, and program staff with respect at all times; harassment, dishonesty, or insubordination may result in removal from the program. Dress Code — I will follow the appearance standards set by my host employer and the program. Cell Phone & Technology Use — I will follow my host employer's policies on personal technology at the worksite. Communication — I will maintain open and timely communication with my career coach or program coordinator, including required check-ins, surveys, and progress reports. Work Completion — I will complete assigned tasks to the best of my ability and treat the internship as a learning experience. Confidentiality — I will keep confidential or sensitive information I encounter at my worksite private. Program Requirements — I will participate in orientations, trainings, career development activities, and reporting requirements outside my internship hours as required. Consequences — I understand that failing to meet these expectations may result in disciplinary action up to and including removal from the program, and may affect my eligibility for future workforce programs.

I am the parent or legal guardian of the student named in this application. I have reviewed this application and the program information provided, and I give my consent for my child to participate in the internship program, including work at an off-site employer location. I have reviewed and agree to the liability waiver, photo/media release, and program expectations on my child's behalf. I understand my child will be expected to meet all program requirements, including attendance, professional conduct, and any additional program activities. I authorize program staff to communicate with my child's school and career coach regarding their participation, progress, and placement. I understand that in an emergency, program staff and/or the host employer will attempt to contact me using the information provided. I consent to my child's participation in any required program orientations, trainings, or career development activities.
As parent or legal guardian, I consent to my child's participation.
